If you do not make an appointment for your child's physical before the school physical date, completed forms must be handed to the school nurse on the first day of school in order to ensure being seen by the school doctor on Septemand cleared to participate in tryouts.Īthletic physical forms filled out and submitted by your family doctor will need to be brought by the parent to the address below and approved by the school doctor before the athlete is allowed to try out and/or participate in any sport. Remember, there is no charge for this service. Athletes are not allowed to try out or participate in a sport without properly completed and approved forms. Parents should complete the parent section of the forms to bring with them to the appointment.
